FINANCIAL SECRETARY GUIDELINES Fraternal Year 2010-2011

NOTE: Please follow this procedure when mailing Form 100s. Thanks.

1. Obtain name of your council Field Agent from the State Directory or call David M. Wellmon,
General Agent at 828-298-3660, so that you may become acquainted with your assigned
agent to better serve your members.

   2. Please follow the below listed procedures, as approved by your State Deputy, and Larry

Mitchell, Supreme Council Membership Department Director:

A. Form 100's - send duplicate copies on all non-insurance transactions to General
Agent Wellmon. Send copy of the Admission Committee Questionnaire, if used,
with New Member 100's. Labels are available from General Agent Wellmon.

B. Form 100's with New Member Annuitv Dlans - advise all new members ( up to
age 83) of this offer for the first year of membership. Also, complete W-9 Forms
(Request for Taxpayer Identification Number). Send both copies of the 100 and the
W-9's witlt flte check (ltome office does not accept cash or coullcil cltecks) and
Form 175FS to General Agent Wellmon. Advise the member that the Field Agent
will deliver the policy.

   C. Death of Members - notify General Agent Wellmon (1-828-298-3660 or Field

Agent, immediately, and send Form 100 to Wellmon.

D. Inquiries I Problems I Ouestions - call your Field Agent immediately should any
member inquire about, or have a problem, or need service. If unable to reach Field
Agent, call General Agent Wellmon.
